Patient Intake Coordinator - Home Health

Silver Haven Home Health is seeking a detail-oriented Home Health Intake Coordinator to manage incoming referrals and assist with determining whether patients are appropriate for home health services. This position will review referral documentation, verify insurance coverage and benefits directly with insurance carriers, identify missing information, and coordinate referrals for timely admission.

Duties

• Receive and review incoming home health referrals.
• Review diagnoses, physician orders, clinical documentation, and supporting records for home health eligibility and medical-necessity requirements.
• Verify insurance eligibility and benefits by contacting insurance carriers and using payer portals.
• Confirm coverage for home health services, effective dates, copays, deductibles, and network status.
• Identify missing or insufficient referral documentation and follow up with physicians, hospitals, and referral sources.
• Escalate referrals requiring clinical determination to the DON/RN as appropriate.
• Communicate with patients regarding referral and admission status.
• Coordinate approved referrals with the clinical team for Start of Care.
• Accurately document referral and insurance information in the EMR.
• Maintain HIPAA compliance and patient confidentiality.

Requirements

• Previous home health intake or referral experience preferred.
• Experience with insurance eligibility and benefits verification.
• Understanding of home health admission requirements and medical-necessity documentation.
• Familiarity with Medicare, Medicaid, and commercial insurance preferred.
• Strong communication, organization, and follow-up skills.
• Experience with home health EMR systems preferred.
• RN/LVN background preferred for positions involving clinical referral review.
