Role Description The Care Coordinator is a full-time, on-site role based in Woodstock, GA. This role is responsible for coordinating in-home care services, including maintaining scheduling continuity and completion by matching seniors with appropriate caregivers and supporting the Director of Operations. Day-to-day activities include scheduling and adjusting caregiver assignments, monitoring service quality, maintaining accurate caregiver and client records, and communicating with clients, families, and caregivers to address questions or concerns. The Care Coordinator works closely with leadership to ensure compliance with company policies and care standards, supports intake and follow-up for new clients, and helps resolve service issues promptly and professionally. - Demonstrated skills in Care Coordination to oversee ongoing services.
- Strong Communication and Customer Service abilities to interact effectively with seniors, families, caregivers, and internal team members.
- Experience or familiarity with Home Care settings, including understanding of in-home care practices and senior support needs.
- Ability to organize schedules, manage multiple cases, and maintain accurate documentation and records.
- Ability and willingness to work in the field, as necessary

- Basic computer proficiency for scheduling, documentation, and communication (e.g., email, care management systems, Microsoft Word).
- Prior experience in senior care, social services, or healthcare coordination is preferred.
- High school diploma or equivalent required; additional education in healthcare, social work, or related fields is a plus.
- Reliable transportation and availability to work full-time on-site in Woodstock, GA.
