Responsibilities:
- Assess client needs and develop individualized care plans to support their goals and well-being
- Coordinate and monitor delivery of services across multiple community and social service agencies
- Maintain accurate and up-to-date client records, case notes, and documentation
- Serve as a primary point of contact and advocate for clients throughout the case management process
- Conduct regular follow-ups and progress evaluations to ensure clients are meeting milestones
- Collaborate with internal teams, external providers, and community partners to ensure holistic client support
- Identify and connect clients with appropriate community resources, programs, and referrals
- Conducts assessments and develops service planning
Requirements:
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred
- Prior experience in case management, social services, or a related role
- Strong knowledge of community resources and social service systems in the Moreno Valley/Inland Empire area
- Excellent communication, organizational, and interpersonal skills
- Ability to manage a diverse caseload and prioritize effectively in a fast-paced environment
- Proficiency in case management software and Microsoft Office Suite
- Compassionate, client-centered approach with a commitment to equity and inclusion
