Job Overview
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their health and well-being while promoting independence. You will work closely with patients, families, and interdisciplinary teams to develop and implement individualized care plans that address the unique needs of each patient.
Duties
- Conduct comprehensive assessments of patients' health status, including vital signs and medical history.
- Develop and implement personalized care plans based on patient assessments and collaboration with healthcare providers.
- Provide skilled nursing care, including medication administration, wound care, and management of chronic conditions.
- Educate patients and families on disease management, medication adherence, and healthy lifestyle choices.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Collaborate with case management teams to coordinate services and resources for patients.
- Document all patient interactions and interventions accurately in electronic health records (EHR).
- Participate in quality improvement initiatives and adhere to infection control protocols, especially in cases involving infectious disease care.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Previous experience in home health or outpatient settings preferred.
- Knowledge of behavioral health practices is a plus.
- Experience in case management or utilization management is desirable.
- Familiarity with Level I Trauma Center protocols is beneficial.
- Ability to assess and manage patients with various conditions, including those requiring PICU or ER-level interventions.
- Strong communication skills to effectively educate patients and collaborate with healthcare teams.
- Ability to work independently while managing multiple patient caseloads effectively.
