Job Summary
We are seeking a compassionate and skilled Registered Nurse for our Home Health division. The ideal candidate will provide high-quality patient care in the comfort of patients' homes, ensuring their health and well-being through comprehensive assessments, individualized care plans, and effective communication with healthcare teams. Duties
- Conduct thorough patient assessments to develop and implement personalized care plans.
- Administer medications and treatments as prescribed, including IV infusions and injections.
- Monitor vital signs and document patient progress accurately in electronic health records (EHR).
- Provide education to patients and families regarding health management, medication administration, and home care techniques.
- Collaborate with interdisciplinary teams to coordinate care for patients with complex medical needs.
- Assist with procedures such as catheterization, tube feeding, and wound care.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
- Participate in discharge planning and case management for continuity of care.
- Provide support for patients with developmental disabilities, memory care needs, or chronic conditions.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Experience in home health or community-based nursing preferred.
- Proficient in using EMR systems such as Epic or Cerner for documentation.
- Familiarity with ICD-10 coding and NCQA standards is advantageous.
- Critical care experience or experience in Level I or II trauma centers is a plus.
- Knowledge of medical terminology, anatomy, physiology, and infection control practices.
- Skills in patient assessment, triage, and acute pain management are essential.
- Experience working with diverse populations including geriatrics, pediatrics, or those requiring hospice & palliative medicine is highly desirable.
- Ability to perform essential nursing skills such as phlebotomy, venipuncture, suturing, and airway management.
- Strong communication skills to effectively educate patients and collaborate with healthcare teams.
