What are the responsibilities and job description for the Quality Intake LVN (Weekend) (Home Health exp required) position at SIGNATURE Health Services?
We are looking for an energetic and skilled quality assurance person to join our team in a home health setting. We need a nurse that is a team player and open to change. We are growing and plan to continue that course, so we need a team that is willing to help us continue that course.
Job Summary: Primary function is to facilitate the referral intake process, coordinate care with the interdisciplinary team. the patient/client, their family, and the referral source.
Job requirements:
- Geriatric Home Health Experience (excluding private duty and hospice)
- HH QA experience
- The understanding of HH coding is a plus
- Nursing skills as defined as generally accepted standards of practice.
- Good interpersonal skills
- Good time management skill and can handle high volume
Education: Graduate of an accredited school of Vocational Nursing
Job Types: Full-time, Part-time
Pay: $30.00 - $35.00 per hour
