Job Summary
We are seeking a dynamic and compassionate LVN Wellness Director to lead our wellness and clinical services within a vibrant senior care environment. Duties
- Develop and implement comprehensive wellness programs and careplans tailored to the needs of seniors and individuals with disabilities, emphasizing holistic health and preventive care.
- Supervise and mentor staff, ensuring adherence to clinical protocols, safety standards, and regulatory requirements.
- Oversee resident assessments, vital signs monitoring, medication administration, and medical documentation using the EMR of Point Click Care.
- Coordinate with interdisciplinary teams to facilitate care planning, case management, and specialized care such as dementia, Alzheimer's, hospice, and palliative services.
- Ensure compliance with infection control practices,
- Conduct or organize regular staff training.
Experience
- Proven experience in diverse nursing environments including hospital medicine and MedSurg.
- Demonstrated expertise in long-term care facilities such as nursing homes or assisted living communities with familiarity in geriatrics and dementia care.
- Strong background working with individuals with developmental disabilities or behavioral health needs; experience in hospice or post-acute care is highly desirable.
- Proficiency in EMR/EHR systems such as Point Click Care or Elder care; knowledge of medical imaging and diagnostic evaluation processes is a plus.
- Skilled in patient assessment techniques including vital signs measurement, anatomy knowledge, physiology understanding, and diagnostic evaluation.
