Job Overview
Sierra View Homes Retirement Community is a senior living community dedicated to providing compassionate, resident-centered care through Skilled Nursing, Assisted Living, Memory Care, and Independent Living services. In this role, you'll oversee resident nutritional care by conducting assessments, developing individualized nutrition plans, ensuring compliance with state and federal regulations, and collaborating with our interdisciplinary team to promote the health and well-being of our residents. Key Responsibilities
- Complete nutrition assessments for residents upon admission and as required.
- Develop, implement, and monitor individualized nutrition care plans.
- Assess and intervene for residents at nutritional risk.
- Monitor weight, lab values, hydration, skin integrity, and other nutrition-related indicators.
- Provide Medical Nutrition Therapy (MNT) and recommend therapeutic diet modifications.
- Review physician diet orders for accuracy and compliance.
- Collaborate with nursing, physicians, therapy, pharmacy, and other interdisciplinary team members.
- Participate in care plan meetings and Quality Assurance & Performance Improvement (QAPI) activities.
- Assist with menu planning and ensure menus meet nutritional, therapeutic, and regulatory standards.
- Monitor food quality, meal presentation, portion sizes, and sanitation practices.
- Educate residents, families, and staff on nutrition-related topics.
- Maintain accurate documentation in the electronic medical record (EMR).
- Ensure compliance with CMS, CDPH, Title 22, HIPAA, infection prevention standards, and other applicable regulations.
- Support state surveys and perform other duties as assigned.
Qualifications
Required
- Bachelor's degree in Dietetics, Nutrition, or a related field.
- Registered Dietitian (RD/RDN) credential in good standing through the Commission on Dietetic Registration (CDR).
- Current California licensure, if required.
- ServSafe Food Protection Manager Certification, or ability to obtain within six months of hire.
Preferred
- One (1) year of experience in long-term care, skilled nursing, rehabilitation, assisted living, or geriatric nutrition.
- Experience using PointClickCare or other electronic medical record (EMR) systems.
- Knowledge of CMS, Title 22, and California long-term care regulations.
- Current CPR certification.

- Excellent assessment, critical thinking, and problem-solving skills.
- Strong communication and interpersonal skills with the ability to educate residents, families, and staff.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Strong organizational, time management, and documentation skills.
- Proficiency with Microsoft Office and electronic medical record (EMR) systems.
Physical Requirements
- Ability to walk throughout the facility and stand, bend, reach, and sit for extended periods.
- Ability to occasionally lift up to 25 pounds.
- Ability to effectively review medical records and use computer systems.
- Ability to communicate clearly with residents, families, physicians, and staff.
Working Conditions
- Work in an indoor healthcare and food service environment.
- May be exposed to kitchen areas and infectious diseases while following infection control protocols.
- Occasional evening or weekend work may be required for meetings, surveys, or special events.
