Position Summary

The Admissions Coordinator serves as the primary point of contact for Skilled Nursing Facility (SNF) admissions and is responsible for coordinating the admission process from referral through resident arrival. This position develops and maintains relationships with hospitals, physicians, case managers, social workers, and community partners to facilitate timely admissions and promote Sierra View Homes' post-acute care services. The Admissions Coordinator collaborates with the interdisciplinary team to ensure regulatory compliance, accurate documentation, exceptional customer service, and positive outcomes for residents and referral partners. The position also provides admissions support for Independent Living and Assisted Living as assigned.

Essential Duties and Responsibilities

Admissions Coordination

  • Serve as the primary intake coordinator for Skilled Nursing Facility admissions.
  • Serve as the primary point of contact for incoming referrals for skilled nursing and post-acute care services.
  • Monitor and review referrals through referral management platforms (e.g., Ensocare, AIDA, and other approved referral systems).
  • Collaborate with the Director of Nursing and Nursing Home Administrator in evaluating referrals and determining admission eligibility.
  • Verify insurance coverage, eligibility, authorizations, and payer requirements for Medicare, Medi-Cal, HMOs, commercial insurance carriers, and other payers.
  • Complete and submit required insurance authorization and referral documentation.
  • Coordinate admissions with residents, responsible parties, and family members.
  • Prepare, review, and complete all admission documentation with residents and/or responsible parties.
  • Enter and maintain accurate resident information within the Electronic Health Record (EHR), including demographic information and face sheets.
  • Ensure all departments receive timely notification of pending admissions and communicate anticipated admission timelines.
  • Collaborate with Nursing, Therapy, Social Services, Business Office, Dietary, Activities, and other departments to ensure a smooth transition into the facility.
  • Respond professionally and promptly to inbound telephone inquiries regarding admissions and services.
  • Assist the Independent Living and Assisted Living programs with admission documentation and coordination as requested.
  • Maintain complete, accurate, and confidential admission records in accordance with HIPAA, federal and state regulations, and facility policies.

Referral Development and Community Relations

  • Develop and maintain strong professional relationships with hospital discharge planners, case managers, social workers, physicians, home health agencies, hospices, and other referral sources.
  • Conduct routine on-site visits to hospitals, physician offices, and community partners to strengthen referral relationships and support admission growth.
  • Participate in community outreach, marketing, networking, and business development activities to increase awareness of Sierra View Homes' services.
  • Represent Sierra View Homes professionally at community events, healthcare meetings, and networking opportunities.
  • Promote the organization's mission, services, and commitment to quality care while fostering positive relationships with referral partners.
  • Communicate regularly with referral sources regarding admission status and available services.

Collaboration and Quality

  • Participate in quality improvement initiatives related to admissions, customer satisfaction, and referral development.
  • Assist with tracking referral activity, admissions, denials, and conversion rates as requested.
  • Work collaboratively with the interdisciplinary team to support continuity of care and successful resident transitions.
  • Perform other duties as assigned.
  • Actively support the mission, vision, values, and strategic goals of Sierra View Homes.

· Attend and actively participate in weekly rehab meetings to support admission planning and care coordination.

Qualifications

Education

  • High school diploma or equivalent required.
  • Associate degree or college coursework in Business, Healthcare Administration, Marketing, or a related field preferred.

Experience

  • Minimum one (1) year of administrative or admissions experience, preferably in a skilled nursing facility, hospital, healthcare organization, or service-oriented environment.
  • Experience with insurance verification, admissions, case management, discharge planning, or referral coordination preferred.
  • Experience with electronic health records and referral management systems is desirable.

Knowledge, Skills, and Abilities

  • Knowledge of Medicare, Medi-Cal, managed care, and commercial insurance authorization processes preferred.
  • Excellent organizational, time management, and multitasking skills.
  • Strong verbal, written, and interpersonal communication skills.
  • Demonstrated customer service skills and professional demeanor.
  • Ability to build trust quickly with residents, families, referral partners, physicians, and healthcare professionals.
  • Ability to develop and maintain positive business relationships with referral sources.
  • Proficient in Microsoft Office (Word, Excel, Outlook) and electronic health record systems.
  • Ability to maintain strict confidentiality in accordance with HIPAA and facility policies.
  • Strong attention to detail, sound judgment, and problem-solving abilities.

Physical Requirements

  • Primarily a sedentary position requiring prolonged sitting, with the ability to stand and walk as needed.
  • Frequent walking throughout the facility and occasional travel between departments.
  • Frequent use of hands and fingers for keyboarding, writing, and operation of office equipment.
  • Occasional reaching, bending, stooping, and lifting up to 10 pounds.
  • Frequent travel to hospitals, physician offices, community agencies, and other referral partner locations.

Cognitive and Sensory Requirements

  • Ability to communicate effectively in person, by telephone, and electronically.
  • Ability to hear, speak, read, and write English sufficiently to perform assigned duties.
  • Visual acuity sufficient to read printed materials and computer screens.
  • Ability to analyze information, exercise sound judgment, prioritize competing demands, and maintain attention to detail in a fast-paced healthcare environment.

Working Conditions

Work is performed primarily in an office and healthcare environment with frequent interaction with residents, families, referral sources, physicians, hospitals, and interdisciplinary team members. Regular travel within the local service area is required to conduct hospital visits, community outreach, and business development activities. Occasional evening or weekend availability may be required to attend networking events, community functions, or to facilitate urgent admissions. The position requires flexibility, professionalism, and the ability to manage multiple priorities while maintaining excellent customer service.

Pay: $60,000.00 - $70,000.00 per year
