Key Responsibilities
- Conduct assessments and develop individualized plans of care across home health, hospice, and palliative patients
- Provide skilled nursing services including pain and symptom management
- Educate patients and families on the hospice philosophy, end-of-life care, and comfort-focused approaches
- Coordinate care with the interdisciplinary team and participate in IDG meetings and care conferences
- Respond promptly to urgent patient and caregiver needs, including crisis intervention
- Supervise and support LPNs and home health aides, including required supervisory visits
- Complete accurate and timely clinical documentation in compliance with hospice Conditions of Participation
- Provide on-call coverage as assigned
What We're Looking For
- Current, unencumbered RN license in New Mexico
- Graduate of an accredited school of professional nursing
- Two or more years of nursing experience preferred; hospice or palliative care experience a plus
- Sound clinical judgment and the ability to work independently in the field
- Compassionate, steady presence with sensitivity to cultural, spiritual, and emotional needs
- Strong communication skills across patients, families, and care teams
- Basic computer proficiency and willingness to learn hospice EMR documentation
- Reliable transportation, valid driver's license, and auto insurance for home visits
