Physical Therapist (PT) | Chicago, IL

We are seeking a licensed Physical Therapist to evaluate, develop, and implement individualized treatment plans for pediatric, adult, and geriatric patients. The PT will provide therapy services, monitor patient progress, collaborate with PTAs and interdisciplinary teams, maintain accurate EMR documentation, and educate patients and families on treatment and discharge planning.
Requirements:
- Active Illinois PT License (or eligibility)
- Experience with PT evaluations, assessments, and treatment planning
- Skilled in therapeutic exercises, mobilization, and PT modalities
- EMR/documentation experience
- Current CPR certification
- Strong communication and patient care skills
