Job description
We are looking to hire a talented and hard-working social worker
Responsibilities of a social worker:
- Responsible for completion of timely assessments and documentation in resident medical records including MDS assessments, admission notes, psychosocial assessments, quarterly and annual assessments, SS progress notes, comprehensive care plan
- Completing all required documentation in the medical record including MDS, care planning, discharge planning and being an integral member of the interdisciplinary team.
- Assist residents and families in addressing concerns
- Provide couseling and education to residents and families about advance directives and health care decision- making
- Coordinate discharge planning efforts for individual residents, make referrals to community service providers, housing, etc
Requirements:
- BS/BA minimum
- Bilingual English/Russian speaking is a plus
- Long term care and short term rehabilitation/nursing home experience
- MDS and Short Term Discharge planning experience required
- Team player with excellent customer service skills required
