Duties
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by observing vital signs and reporting changes promptly using Electronic Health Record (EHR) systems
- Support medication administration following medical instructions and maintaining accurate medical documentation
- Provide specialized care including Alzheimer's, dementia, memory care, hospice, and end-of-life support
- Perform catheter care, wound care, and other medical procedures under supervision or as trained
- Help with light housekeeping tasks such as cleaning, laundry, cooking nutritious meals, and maintaining a safe environment
- Use assistive devices like Hoyer lifts safely to transfer patients and prevent injury
Requirements
- Proven experience working as a Home Health Aide or in assisted living/nursing home settings preferred
- Strong knowledge of medical terminology and home healthcare regulations
- Ability to perform patient observation accurately and respond effectively to emergencies with First Aid and Basic Life Support (BLS) certification
- Experience working with individuals with disabilities or developmental delays is highly valued
- Familiarity with EMR/EHR systems for documentation; HIPAA compliance is essential
- Capable of heavy lifting and physically assisting patients as needed; experience with Hoyer lifts is a plus
- Excellent communication skills to collaborate with healthcare teams and family members
- Valid certification as a CPR provider; valid driver’s license preferred for travel between patient homes
