We began with PRN pharmacy staffing (ShiftRx) and are now launching ShiftOS, a unified operating system for managing the full lifecycle management of the healthcare workforce. By embedding at the shift level, ShiftOS powers a live dataset and automation layer that handles scheduling, compliance, and soon credentialing, and payroll; working towards an entirely intelligence workforce management system. At its core is Holly, an AI agent that learns each organization’s unique workflows to automate scheduling, license tracking, and more—reducing reliance on complex legacy vendors and time spent on manual admin tasks. Healthcare employers are adopting ShiftOS to transform scheduling and workforce management of their workforce, starting with pharmacy teams.

We have top advisors from the AI, data, healthcare spaces, 2x exited founders, the former CTO of Marqueta Nursa, COO Mt. Sinai, Kroger Board Members more. We've recently relocated to SF from ATX, and have >20 enterprise customers in the pipeline with multiple pilots for ShiftOS. Our ShiftRx PRN marketplace is still running currently w/ 1000 pharmacies and 10K clinicians across the US.
This Role
We’re hiring a Founding Engineer to build, ship, and own core parts of ShiftOS. You’ll be an essential founding engineer at the company, working directly with the CEO and CPO, and your code will power the first wave of enterprise deployments. This is a career-defining founding seat.
What You'll Do
- Ship code daily across the stack: React/Next.js, TypeScript, Python, AWS.
- Build distributed backend services and a performant, maintainable frontend.
- Design, deploy, and evolve our cloud infrastructure (AWS, Terraform, CI/CD).
- Integrate AI/optimization systems into real workflows (scheduling, credentialing, payroll).
- Translate customer requirements into simple, scalable solutions.
- Lay the groundwork for observability, testability, and developer velocity.
Who You Are
- 4–8 years building and scaling SaaS products (startup or enterprise).
- Strong IC with fluency in React/Next.js/TypeScript and backend systems in Python.
- Hands-on with AWS, Terraform, Postgres, containerization, distributed systems.
- Excited by AI-native products — you’ve shipped with LLMs, optimization libraries, or are eager to learn fast.
- Gritty, founder-mentality engineer who thrives in ambiguity and wants ownership.
- Bonus: experience with constraint solvers (Google OR-Tools, etc.), healthcare tech, or monorepo tooling (pnpm/Turborepo).
Comp
- Base salary: $150,000-$300,000
- Equity: .5–1% ownership
