What are the responsibilities and job description for the Freedom Connections Aide position at Shiawassee Health & Wellness?
MINIMUM REQUIREMENTS:
EDUCATION:
High School diploma or equivalent.
CERTIFICATION/LICENSURE:
Current CPR, First Aid Certification, and CPI or ability to obtain certification. Valid driver's license.
EXPERIENCE:
One or more years’ experience providing instruction or direct care to individuals with developmental disabilities and/or chronic mental illness.
SUMMARY OF RESPONSIBILITY:
Freedom Connection staff provide health and safety monitoring for individuals during evening/overnight hours so they remain independent while in their own homes. These shifts include weekends and holidays. Applicants must work well on a team and demonstrate good communication skills. Freedom Connection staff frequently communicate with other members of an individual’s treatment team. Freedom Connection may be required to provide some direct care services to individuals during the evening or overnight shift(s). Shift documentation should be completed every time a shift is worked.
