Service Coordinator - Residential Rehabilitation

Schedule: Full time, Day Shift | Job Location Type: In Person

Summary: This is a direct service position requiring knowledge of mental health, community programming and entitlement programs.  Primary responsibilities include the provision and/or coordination of assessing and monitoring client needs, developing service plans, linking clients to services and resources in the community, and advocating for clients as necessary.

Responsibilities:

Enrollment

• Assist clients through program enrollment and orientation processes, including required paperwork.

Coordination of Care

• Provide and/or coordinate the assessment of client needs, development of individual service plans (as assigned), linkages to internal and external services and resources and natural support systems, and advocacy.
• Facilitate effective communication among various program components and with external service providers and natural support systems.
• Address client concerns as they arise, i.e. resource coordination, appointments, transportation, housing, entitlements coordination, debt, money management, etc.
• Transport and accompany individuals to appointments as needed.
• Observe and report symptoms of decompensation to appropriate staff and agencies.
• Plan and facilitate discharges. 

Entitlements

• Obtain and maintain entitlements for clients.
• Maintain copies of social security card, birth certificate, uninsured eligibility information, and other pertinent documents.
• Collect income verification.
• As assigned, coordinate work/wage incentives with entitlement programs to ensure continued eligibility.

Documentation and Service Authorization

• Maintain timely, accurate and complete documentation regarding services and functions, including service delivery, entitlement information and linkages to internal and external services.
• As assigned, obtain service authorizations from the Managed Care Organization (MCO) and monitor service utilization. Appeal denials for services by the MCO.
• As assigned, facilitate the timely development and implementation of individual service plans.
• Develop and coordinate transition plans and program discharge summaries.
• Send status change e-mails as assigned.

Other Duties

• Meet with clients in their homes and other locations to provide face-to-face services and support.
• Engage in ongoing training to improve knowledge of service systems, regulations and procedures.
• As assigned, develop medication plans with clients and assist clients with the management of medications, including but not limited to packing medications with clients, managing and recording changes to medication sheets and plans, and working to ensure that client have an adequate supply of medications at all times. 
• Perform other duties as assigned.

Requirements:

• Work requires the ability to read, write, and speak English and working knowledge of specific populations served, e.g., children and adolescents or adults with mental illness.
• Work requires possession of a bachelor’s degree and 1 year of positively reference relevant work experience or an additional year of relevant work experience in lieu of a bachelor’s degree.
• Ability to obtain additional certifications/trainings as may be required by certain programs (e.g., CPR/First Aid, Infection Control, Food Safety and Handling, and Crisis Intervention/Suicidal Protocol).
• Work requires orientation period to obtain sufficient familiarity with behavioral rehabilitation methods, procedures and techniques, program policies and procedures, and specific patient population.
• Must possess a valid state driver's license. Certain programs and positions may require access to a vehicle during working hours for work related duties, including transporting clients as necessary. When access to a vehicle is required vehicle insurance must name the employee as an insured driver.
• The ability to physically assist clients with daily tasks.
• Good organizational, communication and time management skills.
• Ability to establish rapport and to assist client to identify their needs (i.e., self-care skills, social skills, independent living skills, medication management, mental and somatic health, entitlements, and vocational) to establish appropriate rehabilitation goals and to provide internal and/or community referrals when necessary.
• Working knowledge of Microsoft Office, ability to navigate through applications and utilize keyboard to input required documentation into electronic health record.
• Must be able to work variety of shifts including day, evening and weekend hours and holidays.
