We are seeking a passionate and mission-driven Mobile Case Manager to help expand the impact of Shepherd’s House Ministries across all sites and advance this life-changing work.
The Mobile Case Manager provides trauma-informed, participant-centered support to promote safety, stability, and progress toward housing and well-being. As part of a small mobile team, this role drives into areas throughout the Bend area, engaging participants, assessing needs, developing individualized plans, coordinating services, maintaining accurate documentation, and collaborating with staff and community partners while upholding organizational values and ethical standards.
Essential Duties and Responsibilities:
- Conduct outreach to houseless individuals, building relationships and assessing needs
- Provide participant-centered, trauma-informed case management
- Complete intakes, assessments, and identify barriers and goals
- Develop and maintain individualized service or case plans
- Manage an active caseload with consistent follow-up and support
- Connect participants to housing resources and stability pathways
- Coordinate referrals to healthcare, behavioral health, employment, and social services
- Collaborate with staff, volunteers, and community partners for continuity of care
- Maintain accurate, timely, and confidential documentation in required systems
- Respond to crises appropriately using de-escalation and support connections
Qualifications:
Education & Experience:
- Bachelor’s degree in Social Work, Human Services, Psychology, Behavioral Sciences, or a related field, or equivalent combination of education and relevant lived/professional experience
- Experience providing case management, outreach, or direct services to individuals experiencing homelessness or complex needs
- Knowledge of trauma-informed care and ability to engage participants with empathy and professional boundaries
- Ability to conduct assessments and develop individualized service or case plans
- Experience coordinating referrals and collaborating with internal teams and community partners
- Strong documentation skills, including maintaining accurate and confidential records (e.g., HMIS)
- Strong communication, organizational, and crisis de-escalation skills, with the ability to manage a diverse caseload
Physical Requirements:
- Ability to sit, stand, walk, and move throughout facilities and community locations for extended periods
- Ability to transition frequently between administrative tasks (computer/data entry) and direct participant interaction
- Ability to bend, reach, stoop, and lift up to approximately 25 pounds on an occasional basis
- Ability to use a computer, phone, and other standard office equipment for documentation, communication, and reporting
- Ability to travel locally between program sites or outreach locations, as required by the role
- Ability to visually and audibly assess situations, read documentation, and communicate clearly with participants, staff, and partners
