Role Description
Shepherd’s Gate Health is seeking a passionate and relational Health Coach to support participants in our You Are Food Farmacy program, an innovative and unique Food is Medicine initiative serving the greater Rock Hill and Fort Mill community. The You Are Food Farmacy combines produce prescriptions, health coaching, and community health support to help individuals living with chronic conditions build sustainable healthy habits.
Role Overview The Health Coach will support participants enrolled in our 12-week Food Farmacy program by providing coaching sessions focused on nutrition, lifestyle habits, and behavior change.
Coaches help participants translate healthy intentions into practical daily habits that support long-term health. Sessions may occur in person during produce pickup, by phone, or virtually.

Participant Intake
- Nutrition habits
- Lifestyle patterns
- Barriers to healthy eating
- Personal health goals
Health Coaching Sessions including
- Meal planning
- Cooking with produce box ingredients
- Reading nutrition labels
- Portion control
- Reducing sugar and sodium
- Grocery shopping strategies
- Building sustainable lifestyle habits

Health Coaches reinforce the affirming spirit of the program by reminding participants:

- Documentation: Maintain simple session notes to support program evaluation and outcome tracking.
Collaboration

- Clinical referral partners when appropriate
Qualifications
Preferred candidates may include:
- Registered Dietitian (RD/RDN)
- National Certified Health Coach or Wellness Coach
- Experience in Chronic disease prevention
- Background in Public Health, Nutrition, Nursing, or Health Education OR
relevant experience in: ● Lifestyle coaching ● Community health programs
Ideal Candidate Traits:
● Warm and relational ● Encouraging and non-judgmental ● Passionate about food as medicine ● Skilled at motivating behavior change ● Comfortable working with diverse communities Experience supporting individuals managing conditions such as diabetes, hypertension, or obesity is highly valued.
Compensation:
Compensation Independent Contractor (1099) $30 per hour
Estimated time commitment: Approximately 8 hours per participant per 12-week program cycle.
Flexible scheduling.
Location:
Location Hybrid role serving participants in Rock Hill, SC; Fort Mill, SC; and the York County area. Some in-person engagement during produce pickup days may be requested.
