Director of Social Services
Assists residents and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care by performing the following duties.
Responsibilities include the following. Other duties may be assigned.
- Works in close collaboration with physicians and other heath care personnel in resident evaluation and treatment for further their understanding of significant social and emotional factors underlying resident's health problem.
- Coordinates discharge planning process
- helps resident's family through individual or group conferences to understand, accept, and follow medical recommendations.
- coordinates and actively participates in weekly interdisciplinary care plan meetings.
- prepares resident histories, care plans, and reports.
- participates in planning for improving health services by interpreting social factors pertinent to development of program.
- maintains confidentiality of all resident, staff and organization information.
Education Director of Social Services
Bachelor's degree from 4 year college or university preferred with LSW license. Or two years of experience in long term care as Social Service designee.
Licenses
Licensed Social Worker preferred
