Overview
We are seeking a personable and professional Physical Therapist to join our healthcare team. This ideal position will provide flexible schedules and independence with patient care mainly working with seniors living in community settings. This role is for physical therapists who are looking for a more relaxed, go-at-your-own pace approach to treating. We are not Home-Health (maximum 60 days in-home care). We are in-home outpatient, meaning we see more patients in a day, but our documentation is much more simple and less time consuming. Because we are outpatient providers (mainly using Medicare Part-B), we can see patients for weeks, months and years depending on the diagnosis. This allow us to actually treat our patients as long as we needed, and we are badly needed in the homes of many mobility-challenged seniors.
Responsibilities
- Assess patient conditions through evaluations and develop individualized care plans based on clinical findings and patient needs.
- Navigate basic EMR system
- Be dynamic in approach to treatment
- Monitor patient progress regularly and adjust treatment strategies accordingly to maximize recovery.
- Educate patients and their families on injury prevention, home exercises, and community resources to support ongoing health management.
Requirements
- Valid licensure as a Physical Therapist with relevant state or national credentials.
- Strong knowledge of anatomy, physiology, and medical terminology relevant to physical therapy practice.
- Experience with developing comprehensive treatment plans that include discharge planning and patient education.
- Familiarity with EMR systems for documentation and patient record management.
- Demonstrated expertise in managing diverse patient populations including geriatrics, orthopedics
- Excellent communication skills for effective collaboration with healthcare teams and educating patients about their care.
