Responsibilities
- Conduct comprehensive assessments of patients’ physical conditions, including physiology and anatomy evaluations
- Develop and implement individualized care plans tailored to each patient’s needs, focusing on discharge planning and recovery goals
- Provide physiotherapy treatments such as trigger point therapy, orthopedics, and rehabilitation exercises for acute care and outpatient settings
- Assist patients with mobility, pain management, and functional improvements through evidence-based techniques
- Collaborate with interdisciplinary teams to coordinate discharge planning, home & community care services, and follow-up care
- Maintain accurate documentation of patient progress using Electronic Medical Record (EMR) systems
- Educate patients and their families on physiotherapy techniques, injury prevention, and health maintenance strategies
Skills
- Strong knowledge of physiology and human anatomy relevant to diverse patient populations
- Proven experience in discharge planning, acute care, home health, outpatient clinics, and hospital environments
- Expertise in care plan development for pediatrics, geriatrics, orthopedics, and general patient care
- Proficiency with EMR systems for documentation and patient record management
- Excellent understanding of medical terminology and healthcare protocols
- Skilled in physiotherapy techniques including trigger point therapy and orthopedic rehabilitation
