Sheltering Arms Institute, a collaboration with VCU Health is scheduled to open its doors in late spring 2020. Sheltering Arms Institute combines inpatient beds from Sheltering Arms Hospital and VCU Health to create a multi-million dollar state-of-the-art destination hospital and is a leader in rehabilitation medicine throughout the mid-Atlantic region. The organizational values and transdisciplinary approach to care catalyze improvements in patient outcomes and the realization of Sheltering Arms Institute’s vision to reinvent rehabilitation. SAI leaders and team members align behaviors with this approach to build team cohesion and to foster a culture that team members, patients, families and community members experience during their interactions with the Institute.

 

Sheltering Arms Institute is seeking a LPN to join the team. We currently have an opening for night shift 7pm-7am.   The LPN is responsible for the provision of rehabilitative nursing care for assigned patients as planned and coordinated by the RN and as patient needs are identified. Follows up to ensure appropriate standards of care are provided.

 

Performance Responsibilities

Transdisciplinary Care

  • Ensures all team members are oriented to the transdisciplinary culture and expectations on the unit. Clearly communicates the structure, practice, and expectations of the Sheltering Arms Institute care model.
  • Provides support and coaching to team members to help them transition into and maintain a philosophy of transdisciplinary teamwork through role release and maintaining the highest standards of patient satisfaction.
  • Models transdisciplinary behavior and leadership, assists in setting unit expectations regarding behavior, assists families and patients with their needs to foster a more collaborative, meaningful, and engaged patient care experience.
  • Facilitates access to and engagement with formal and informal interprofessional education for all team members, including learners. 

 

Assessment, Planning and Implementation under direction of RN

  • Responsible for the provision of rehabilitative nursing care for assigned patients as planned and coordinated by the RN and as patient needs are identified.
  • Follows established policies and procedures, MD orders, and standards of care while providing care.
  • Assists with assessment, under the supervision of a RN, by accurately collecting data, but is not limited to: patient's physical assessment, pain level, age, social, mental status, cultural, spiritual needs, safety needs.
  • Assists with evaluation of patient's learning needs, readiness, and capacity and educational level, teaching and discharge needs.
  • Assists in identifying rehab goals.
  • Ensures pertinent patient information is shared with RN and Nurse Coordinator/Relief Charge.

 

Education and Patient Teaching

  • Acts as a patient/family advocate for education of the disease/illness.
  • Documents patient's education and level of understanding and response to teaching regarding disease process and medications.
  • Adapts communication style to meet age specific and identified learning needs of the patient. Assists new team members/students, co-workers with procedures and/or unit orientation.

 

Leadership and Professionalism

  • Maintains professionalism, licensure, and education record, attends all mandatory education and stays current on TJC/CMS regulations as well as hospital policies and procedures.
  • Utilizes time effectively, responds to requests efficiently and effectively.
  • Supervises and educates Nursing Techs.
  • Acts as team player and as a liaison to other departments.
  • Takes initiative to improve knowledge, skills and abilities, furthering education from LPN to RN.

 

Medication Administration

  • Demonstrates competency in all medication administration, IV therapy and blood product transfusion according to hospital policy.
  • Recognizes unique responses to medication based upon age of patient.
  • Evaluates patient's response to medication/IV and accurately documents.
  • Consults dietitian for food/drug interaction as appropriate.
  • Always uses appropriate resources (pharmacy, drug manuals) to ensure proper administration. Recognizes and reports adverse drug reactions.
  • Shares pertinent patient information with RN and Nurse Coordinator/Relief Charge.

 

Documentation

  • Ensures nursing documentation reflects holistic care and is accurate and completed on time and contains pertinent information regarding patient's conditions, status changes, response to treatment and progress towards outcome goal.
  • Documents patient events/occurrence reports in a thorough and concise manner. 
  • Evaluates the effectiveness of comfort interventions (including all PRN medications given) within one hour.
  • Consistently uses approved abbreviations when documenting in the patient record.
  • Completes and ensures FIMS scoring is accurate and completed on time.
  • Documents patient/family education.

 

Assuring Quality of Care

  • Demonstrates competency in interpreting, processing, clarify physician orders.
  • Maintains records, checking charts throughout the shift.
  • Handles patient/family concerns and requests.
  • Orients patient/family to rehab unit including but not limited to reviews schedule, routines, and expectations.
  • Ensures consent has been obtained before any care is rendered. Ensures confidentiality (HIPAA) is reviewed and a copy is provided.

 

Performs other duties and participates in special projects

  • Performs other duties as assigned and/or participates in special projects in order to support the mission of Sheltering Arms Institute and departments.
  • Provides assistance to team members.
  • Accepts alternate assignments as required.

 

Qualifications and Skills

  • 1 year of experience as a LPN is preferred

 

Education/Certifications

  • Graduate of an approved LPN program required.
  • LPN - Licensed Practical Nurse, in good standing
  • AHA BLS HCP Certification or equivalent
  • Furthering education as RN preferred
