Responsibilities
- Provide safe, competent nursing care in accordance with Illinois Nurse Practice Act standards and facility policies.
- Perform comprehensive assessments and monitor residents’ physical and emotional health.
- Administer medications and treatments accurately while maintaining proper documentation.
- Collaborate with physicians and interdisciplinary healthcare teams to develop discharge plans and coordinate ongoing care for individuals with complex needs, including dementia and developmental disabilities.
- Maintain infection control protocols and follow sterile processing procedures to ensure a safe environment for residents and staff.
- Support residents in achieving the highest level of independence through health education, skill-building, and daily living assistance.
- Respond promptly to changes in residents’ conditions and implement appropriate interventions.
- Document all nursing care, observations, and communications clearly and accurately.
- Provide compassionate, person-centered care in a home-like environment.
- Participate in quality assurance, safety initiatives, and ongoing professional development.
- Communicate effectively with residents, families, and team members to promote continuity of care.
- Ensure proper storage, handling, and accountability of medications and medical supplies.
Experience & Requirements
- Current and valid Illinois RN or LPN nursing license
- Active Basic Life Support (BLS) certification
- Strong general nursing skills aligned with Illinois Nurse Practice Act standards
- Experience supporting individuals with developmental disabilities or dementia (preferred but not required)
- Ability to work collaboratively with interdisciplinary healthcare teams
- Commitment to maintaining safety, infection control, and high-quality resident care

Pay: $34.00 - $36.75 per hour
