Homeless Case Manager

We are looking for a Case manager to assess and guide the homeless, with compassion and informed accountability methods. Case manager duties include determining program eligibilty, coordinating self assessment, personal goal setting, implementing, monitoring and evaluating actions required to meet the client’s housing and human service needs. Knowledge of local human resources is a plus.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
• Handle case assignments, guide clients toward healthy goal setting, review case progress and work with staff to determine case needs and closure.
• Help clients self assess their situation and progress toward self sufficiency.
• Facilitate multiple care aspects (case coordination, information sharing, etc)
• Help clients make informed decisions by acting as their advocate regarding their stay at the Shelter and finding a positive path going forward.
• Educate clients relative to services provided by social services, housing, health and governmental agencies
• Conduct weekly meetings and interact with clients to keep track of their progress and to ensure progress
• Record cases information, complete accurately all necessary forms and produce statistical reports
• Assess and address clients need to become a successful resident
• Adhere to professional standards as outlined by protocols, rules and regulations

Job Types: Part-time, Full-time

Pay: $14.00 - $15.50 per hour
