What are the responsibilities and job description for the Public Health Coordinator - Peer Navigator (Health Services) position at Shelby County Government?
Position Summary
This position is funded by a grant for a duration of time as defined by the grantor.
Under the manager's supervision, this role works with substance use participants in Shelby County's high-impact drug areas to assist clients in obtaining treatment, conducting harm reduction outreach, and collaborating with community partners to enhance health outcomes.
Pay Grade: 51
Salary Commensurate with Experience and Education
Minimum Qualifications
Duties and Responsibilities
1. Meets and screens participants for navigation services and works with hospital staff to address intervention services.
2. Refers to patients for substance use disorder and other treatment services and medication assistance treatment programs.
3. Visits and maintains records of patients admitted for recovery services and assesses quality and program availability.
4. Collects and analyzes data to complete deliverables and reports and develops a reporting system for management.
5. Builds community awareness and support through promotion, media, Shelby County Opioid Speakers presentations, informational materials, talking points, and curricula.
6. Participates and assists in coordinating and ensuring effective implementation of outreach events such as “START: Strategic Team Accessing Recovery Treatment”.
7. Provides harm reduction and resource information for treatment of substance use disorder.
8. Works collaboratively with other Peer Navigator Coordinators in Shelby County regarding changes in trends of substance users, demographics, etc.
9. Participates in substance abuse task force bimonthly meetings and Opioid weekly meetings.
10. Participates with Syringe Service Programs (SSP) programs to assist in navigating patient treatment services.
11. Identifies and provides education to community stakeholders such as residents, schools, universities, neighborhood associations, professional organizations, churches, and community-based agencies to inform and engage their participation.
12. Attends meetings and community events to promote awareness of opioids.
13. Stays abreast of current counseling techniques and best practices.
14. Performs other related duties as required or directed.
