Position Summary
The Youth Coordinator is responsible for providing clinical and supportive services to youth experiencing behavioral, emotional, and environmental challenges, including those from high-risk or unstable home environments. This role combines case management, therapeutic support, and community-based intervention, with a strong emphasis on school collaboration and youth engagement. Will serve as a licensed professional who provides structured interventions, advocacy, and mentorship while coordinating care between schools, families, and community resources.
Key Responsibilities
- Provide individual supportive counseling and skill-building sessions for youth using trauma-informed and strength-based approaches
- Complete assessments, service plans, and progress notes in compliance with agency, state, and payer requirements
- Deliver case management services, including linking youth to community resources, healthcare, and educational supports
- Coordinate closely with school personnel (teachers, counselors, administrators) to support academic and behavioral success
- Provide transportation for youth to school, appointments, extracurricular activities, and other approved services
- Assisting youth in engaging in sports, prosocial activities, and community involvement
- Develop and implement individualized service/treatment plans based on client needs
- Monitor client progress and adjust interventions accordingly
- Maintain consistent communication with families/guardians and involved systems
- Identify and respond to crisis situations, including risk assessment and appropriate reporting (mandated reporting as required)
- Maintain timely and accurate clinical documentation that meets Medicaid and regulatory standards
- Participate in treatment team meetings, case reviews, and supervision
Required Qualifications
- LSW or LPC
- Bachelor’s degree in social work
- Experience working with youth, adolescents, or at-risk populations preferred
- Knowledge of trauma-informed care, behavioral health, and family systems
- Valid driver’s license with a clean driving record
Competencies
- Ability to provide evidence-informed interventions appropriate to youth populations
- Strong understanding of case management and care coordination
- Knowledge of mandated reporting laws and ethical responsibilities
- Crisis intervention and de-escalation skills
Physical Requirements
- Ability to transport clients safely
- Ability to engage in youth activities (recreational, school events, etc.)
- Ability to respond effectively in crisis situations
Pay: $35.00 - $40.00 per hour
