Case Manager

Position Summary

The Case Manager is responsible for coordinating services and connecting clients with the resources necessary to support their recovery, stability, and overall well-being. The Case Manager serves as the primary liaison between the client, treatment team, and community providers to ensure clients receive timely access to medical, behavioral health, housing, employment, financial, educational, legal, and other supportive services.

This position focuses on advocacy, service coordination, referrals, and ongoing follow-up to help clients overcome barriers to treatment while promoting successful recovery and community integration.

Essential Duties and Responsibilities

  • Maintain an assigned caseload and provide comprehensive case management services.
  • Complete assessments to identify client needs, strengths, barriers, and service goals.
  • Develop and update individualized service plans in collaboration with the treatment team.
  • Coordinate services between clients and community agencies.
  • Assist clients in obtaining:
  • Medical and dental care
  • Psychiatric and behavioral health services
  • Housing and emergency shelter
  • Employment and vocational services
  • Educational opportunities
  • Transportation
  • Food, clothing, and financial assistance
  • Government benefits and insurance
  • Legal and court-related services
  • Recovery support services
  • Make referrals and monitor follow-through with outside providers and agencies.
  • Advocate on behalf of clients to reduce barriers to treatment and recovery.
  • Coordinate communication with probation officers, courts, hospitals, primary care providers, employers, schools, child welfare agencies, and other community partners as appropriate.
  • Monitor client progress toward goals and revise service plans as needed.
  • Assist clients in developing independent living skills and increasing natural support systems.
  • Provide crisis support and facilitate emergency referrals when appropriate.
  • Participate in treatment team meetings and case staffing.
  • Maintain timely, accurate, and complete documentation in the electronic health record.
  • Protect client confidentiality in accordance with HIPAA, 42 CFR Part 2, and agency policies.
  • Maintain productivity standards established by the organization.
  • Perform other duties as assigned.

Knowledge, Skills, and Abilities

  • Knowledge of behavioral health and substance use disorder services.
  • Knowledge of local community resources and referral processes.
  • Strong organizational, problem-solving, and time-management skills.
  • Ability to build professional relationships with clients and community agencies.
  • Excellent verbal and written communication skills.
  • Ability to advocate effectively for clients.
  • Ability to work independently while collaborating with a multidisciplinary treatment team.
  • Proficiency with electronic health records and Microsoft Office applications.

Minimum Qualifications

Education

  • High School Diploma or GED required.
  • Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or a related field preferred.
  • Valid Ohio Driver's License.
  • Proof of automobile insurance meeting agency requirements.
  • CPR/First Aid certification (or ability to obtain upon hire).
  • CPI/De-escalation certification (or ability to obtain upon hire).

Job Type: Full-time

Pay: From $17.50 per hour
