The stated pay scale reflects the range as defined by the collective bargaining agreement between Sharp HealthCare and Sharp Professional Nurses Network, United Nurses Associations of California/Union of Health Care Professionals, NUHHCE, AFSME, AFL-CIO. Placement within the range is based on years of RN experience.
What You Will Do
The Limb Preservation Navigator's role is to be a patient advocate providing patient and their families with a point of care approach and guiding them through the healthcare system providing information regarding services, education, funding, and access to care. The Navigator ensures service access and meets community care needs through quality assessment, triage, service delivery, and care coordination of clinical services between key external constituencies and Sharp.
The Navigator will support Sharp Grossmont Hospital in multi-service referral, program development, and quality care management. Collaborate with the multidisciplinary team to support timely limb and/or wound screening, assessment, and supportive care across the healthcare continuum. The Limb Preservation Navigator will work to reduce anxiety and frustration with coordinating care and improve patient satisfaction with the health care system. The Navigator will facilitate increased quality and cost-effective use of health care resources, and assess program effectiveness, outcomes, and opportunities for program expansion.
Required Qualifications
Maintains on-going communication with multi-disciplinary team members regarding patients' status and follow-up.
Documents consultation, recommendations, and interventions.
Coordinate or conduct limb preservation health education classes with individuals and groups.
Identify target population by utilizing community service data
Utilize data in the evaluation and improvements of the Limb Preservation Program.
Report program progress and effectiveness on a monthly and annual basis.

Required Qualifications
- Nursing diploma.
- Previous clinical experience with wound care, case management, or patient education.
- Experience facilitating the design, implementation, and evaluation of health care promotion programs and or community development.
- California Registered Nurse (RN) - CA Board of Registered Nursing
- AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association
- Bachelor's Degree in Nursing
- Master's Degree in Nursing
- Experience with patients with diabetes, wound care, or case management.
- Demonstrates ability to work cooperatively within the framework of the interdisciplinary team. Participates in and provides on-going communication with other team members including, but not limited to team conferences, supervision meetings, and direct patient/family interactions.

- Develop strong community outreach connections and referrals.
Coordinate or conduct limb preservation health education classes with individuals and groups.
Identify target population by utilizing community service data
- Implements plan of care and strategies to meet the anticipated clinical, quality, and financial outcomes. Refers to internal resources of the hospital that can assist patients and families. Refers to basic community resources. Provides referrals to patients/families for ongoing assistance. Encourages patients and family participation in the decision-making process. Protects patient privacy by maintaining confidentiality.
- Track patient referrals and quality outcomes.
Utilize data in the evaluation and improvements of the Limb Preservation Program.
Report program progress and effectiveness on a monthly and annual basis.

- Able to be innovative and creative in order to develop strategies that successfully meet the needs of a diverse patient, family, and provider populations required.
- Demonstrate competence in team building, conflict resolutions, and interpersonal effectiveness required.
- Excellent computer application skills preferred.
