Location: Plymouth, MA
Primary Individual Functions
· Provide direct care support to disabled Individuals with physical challenges residing in a private Shared Living Home (similar to an Adult Foster Home) at home and in the community
· Assist with transportation, wheelchair transport, personal care, shopping, medical appointments, recreational and therapeutic activities, ADLs, and respite.
· Follow and implement all safety guidelines, procedures, and assessment results to ensure that Individual’s health, personal, and safety needs are met at all times.
· Promote and teach human rights and self-advocacy skills
· Participate in all necessary training, obtain required certifications, and perform non- skilled nursing tasks (training provided), such as tube feedings, as needed.
· Assist with implementation of ISP goals and follow established therapeutic guidelines including Positive Behavior Supports and Trauma Informed Approaches.
· Follow SLC and funding agencies protocols and policies, adhere to reporting requirements, and maintain all required documentation
· Facilitate opportunities for community membership and civic activities
· Promote a healthy life style and work on the development of life, communication, and social skills.
· Promote the development of meaningful relationships and friendships
· Participate in meetings as required and assist in the facilitation of therapeutic and recreational agency activities such as groups, classes, etc.
· Support opportunities for meaningful work/employment
· Assist with crisis intervention when necessary and appropriate
· Be available to work afternoons, evenings, holidays, and/or weekends as agreed upon
Education
Minimum High School Diploma or an equivalent document. Experience in Human Services/Direct Care required.
Skills/Qualities/Knowledge
· Must hold a valid driver's license and clean driving record, and be willing to drive Individuals.
·Must share the belief that people with impairments have the ability to direct their own care, live interdependently or in shared living situations
· Works to reflect the mission and principles of Shared Living Collaborative and DDS.
· Understands the nature, dynamics, and support needs of Shared Living Households
· Ability to maintain a confidential working relationship
· Demonstrates solid interpersonal skills and is reliable, consistent and dependable.
· Must demonstrate excellent knowledge related to all essential safeguards and procedures such as fire safety, environmental and street safety.
· Knowledge of Trauma Informed Approaches and Positive Behavior Supports and ability to apply a variety of therapeutic approaches and strategies
· Exhibits good judgment and decision making skills even in challenging situations and. demonstrates creative problem-solving skills.
· Works well independently and as part of a team.
· Demonstrates initiative, commitment, and positive energy.
· Exhibits ability to complete all necessary administrative tasks such as visitation notes and incident reports in a timely fashion.
· Ability to perform physical tasks such as lifting and providing assistance with mobility
· Demonstrates personal awareness/self-reflection related to work issues and performance and is committed to personal and professional growth.
