What are the responsibilities and job description for the CILA House Manager/DSP position at SHAMROCK SERVICES?
PURPOSE OF YOUR JOB POSITION
Provide support, training, and assistance in the client's individual service plan. You are
also to respect the privacy, choice, and dignity issues while providing these services.
MAJOR DUTIES AND RESPONSIBILITIES
1. Shall provide training to individuals or group as needed/required by training
plan(s). Shall participate in Community Support Team process.
2. Shall provide assistance in budgeting process for each individual.
3. Shall provide intermittent supervision.
4. Shall assist in planning and provide transportation to/from community activities -
including routine Activities of Daily Living and special functions (social or
otherwise).
5. Shall provide documentation as required: training, incident reporting, drills, etc.
6. Shall submit requisitions for materials, equipment, etc. as needed.
7. Shall report maintenance needed (Building and Vehicle).
8. Shall reinforce all house rules and regulations on a day-to-day basis (including
safety). ~
9. Shall attend all in-service sessions, workshops, and conferences as required.
10.Shall assume additional duties as assigned or required.
11. Shall keep all information concerning clients confidential.
EDUCATIONAL REQUIREMENTS
Must possess, as a minimum, a high school diploma or equivalent. Must complete
required Direct Support Person classes.
ESSENTIAL FUNCTIONS
Must be able to accept change of job position/or work schedule resulting in overtime
without prior notice due to the health care provided by our facility to persons with
developmental disabilities.
Must attend all in-service training and staff meetings as required by the GILA
Coordinator/QIDP or Administrator.
Must have knowledge of and adhere to client rights and regulations regarding privacy
issues.
Follow home vehicle policies regarding cleanliness, no smoking, recording of mileage,
etc.
Assist in training new personnel regarding the house, its policies and procedures, and
to his/her job position and duties.
Required to assist non-ambulatory clients in transferring ( equal to or greater than fifty
(50) pounds) for short duration.
Required to assist partially ambulatory clients in walking using gait belt.
Required to assist clients with their self~care skills such as bathing, toileting, eating, etc.
Must not have an infectious disease that would pose any undue risk of infection to
themselves, and other employees, clients, or visitors. Illinois Administrative Code
Chapter 1, Section 150.675.
Must be screened by finger-printing to assure no listing of convictions of previous
crimes or abuse.
Must provide Agency with health inventory which includes, immunization11,status, history
of infectious condition, hepatitis, dermatologic conditions, or chronic draining infections
or open woods.
Must be at least 18 years of age, of temperate habits and good moral character,
honest, reliable and trustworthy.
Must be able to speak and understand the English language or a language understood
by a substantial percentage of the Agency's clients.
Must provide evidence of employment or occupation, if any, and residence for two
years prior to initial employment as a nursing assistant.
Must demonstrate the required level of education competency through testing as set
forth by Federal and State standards.
Required to have a valid driver's license to operate facility vehicle and to be considered
insurable based on criteria by the facility insurance carrief.
Must complete shopping for facility.
Must schedule/document and transport residents to medical appointments.
Must supervise staff.
Must assist with 9ompletion of assessments, consents and all other documentation as
designated.
Pay: $18.60 - $19.10 per hour
