Hospice Social Worker and Bereavement Coordinator - Dual Salaried Position

Responsibilities:

• Assess the emotional and psychological needs of patients and their families, and develop personalized bereavement plans to address these needs

• Provide counseling and support to patients and their families, including individual and group therapy sessions

• Facilitate support groups and educational programs for patients and their families

• Collaborate with the interdisciplinary team to develop and implement comprehensive care plans

• Educate patients and their families on end-of-life care options and advance care planning

• Maintain accurate and timely documentation of patient interactions and services provided

• Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements:

• Master's degree in Social Work (MSW) from an accredited program

• Licensure as a Licensed Clinical Social Worker (LCSW) in the state of Tennessee

• Ability to work independently and as part of a team

• Excellent communication and interpersonal skills

• Ability to maintain confidentiality and handle sensitive information

• Strong organizational and time management skills

• Ability to travel within a designated service area
