Duties
- Conduct comprehensive patient assessments to evaluate emotional, social, and behavioral health needs within hospice care settings.
- Develop and implement individualized care plans that incorporate clinical counseling, behavioral therapy, and family support strategies.
- Provide individual and group counseling sessions focused on grief, loss, addiction counseling, or mental health challenges such as PTSD or dementia-related issues.
- Collaborate with interdisciplinary teams to coordinate services including crisis intervention, case management, and post-acute care planning.
- Maintain accurate medical documentation in compliance with HIPAA regulations and ensure timely updates of medical records using Electronic Medical Record (EMR) systems.
- Assist with intake procedures for new patients and facilitate communication with healthcare providers regarding diagnosis codes (ICD-10/ICD-9), CPT coding, and other medical terminology.
- Offer support for families dealing with Alzheimer's care, working with individuals with developmental disabilities or autism spectrum disorders as needed.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited program; licensure or certification required per state regulations.
- Proven experience in hospice care or home & community care environments; inpatient or nursing home experience is highly desirable.
- Strong knowledge of medical documentation standards, HIPAA compliance, ICD coding (ICD-10/ICD-9), and EMR systems.
- Familiarity with clinical research methodologies and data collection techniques relevant to behavioral health and palliative care settings.
- Expertise in child & family counseling, crisis management, cognitive behavioral therapy (CBT), motivational interviewing, and psychotherapy modalities.
- Ability to work effectively with diverse populations including seniors, children, individuals with disabilities or developmental disabilities, and those requiring Alzheimer's or PTSD care.
- Excellent communication skills for intake assessments, diagnostic evaluations, group therapy facilitation, and interdisciplinary collaboration.
