What You’ll Do:
- Lead and oversee all social services functions within the facility
- Assess residents’ psychosocial needs and develop individualized care plans
- Provide guidance and support to residents and families during transitions
- Coordinate discharge planning and community resource connections
- Collaborate with interdisciplinary teams to ensure holistic, resident-centered care
- Ensure compliance with state and federal regulations
- Maintain accurate documentation and participate in care conferences
- Bachelor’s Degree in Social Work or Human Services field (Sociology, Psychology, Rehabilitation Counseling, Special Education, etc.)
- Previous experience in long-term care or healthcare setting preferred
- Strong leadership, communication, and organizational skills
- Ability to manage multiple priorities in a fast-paced environment
