Job Summary
We are looking for a Home Care Aide to join our team! You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus. Reliable transportation a MUST
Responsibilities
- Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
- Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
- Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor
- Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
- Do the clients shopping or accompany them when they shop
- Engage patients in activities
- Act quickly and responsibly in cases of emergency
- Plan and prepare meals with assistance from the client (when they are able)
Skills
- High School Diploma or GED
- Proven experience as a caregiver
- Excellent knowledge of emergency response and first aid (CPR)
- Willingness to adhere to health and safety standards
- Respectful and compassionate
- Good time management skills
- Outstanding communication and interpersonal skills
- Strong ethics
- Physical endurance
Schedule
- Flexible
- Monday to Friday
- Weekends as needed
Shift Availability
- Day shift (preferred)
Ability to commute / relocate
- Reliable transportation
- Willing to commute or planning to relocate (required)
