Job Overview
We are looking to add a dedicated and energetic Physical Therapist to join our outpatient rehab team. Responsibilities
- Conduct thorough patient evaluations to determine physical therapy needs, including assessments of physiology, anatomy, and medical history
- Develop and implement individualized treatment plans focused on patient rehabilitation goals across orthopedic, neurological, pediatric, and geriatric populations
- Administer various therapeutic interventions such as trigger point therapy, mobility exercises, manual therapy techniques, and patient education
- Document all patient interactions accurately in electronic health records (EHR) systems following clinical documentation standards
- Monitor progress regularly and adjust treatment plans based on patient response and evolving needs
- Educate patients and their families on care plans, home exercises, and community resources to promote ongoing recovery
Requirements
- Valid license to practice as a Physical Therapist in the relevant jurisdiction
- Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques
- Experience working with geriatric patients, pediatrics, neurology cases, orthopedics, or outpatient rehabilitation centers is highly desirable
- Ability to develop comprehensive care plans and discharge strategies tailored to individual patient needs
- Proficiency in electronic health records (EHR) management and EMR systems for accurate documentation
- Excellent client communication skills with the ability to educate patients effectively about their treatment plans
- Knowledge of outpatient clinic operations and home & community care practices is advantageous
