SCHEDULE: Full Time, 8 HR Day Shift,
Position Summary
The Hospice Social Worker is responsible as a member of the Interdisciplinary Team to provide supportive services to meet patient/family/caregiver social, emotional, and economic needs in relation to the impact of a terminal illness, including assisting with bereavement support as indicated. May also participate in the provision of grief counseling/stress management services to hospice staff, volunteers, and others within the community. Responsible to represent HOSG and its services in a competent, professional manner consistent with the organization’s culture of service and teamwork.
Knowledge, Skills & Abilities
Hospice office, patient homes, nursing homes, inpatient hospice unit, and various community settings. Primarily weekdays, but occasionally requires long, irregular, or flexible work hours including some weekend or evening hours when caseload is high or when special Hospice events are scheduled. Frequent travel within service area with occasional overnight out of town educational workshops, approximately 1-2X per year. Periods of moderate to high levels of stress and emotionally difficult situations. Physical work may require sitting, standing, bending, and lifting or pushing objects, usually with a 25 lb maximum. Ability to operate basic office machines such as telephone, fax, copier, and computer required.

Knowledge, Skills & Abilities
- Required Master of Social Work degree from a school of social work accredited or approved by the Council on Social Work Education
- Required licensure by the State of Georgia – LMSW, with LCSW preferred
- Minimum of two years of varied Social Work experience preferred (prefer prior experience in Hospice/Palliative Care)
- Demonstrated understanding of and adherence to the hospice philosophy and ability to work as member of interdisciplinary team in meeting the unique needs of the terminally ill
- Certification in hospice/palliative care strongly preferred
- Must possess strong skills in psychosocial assessment, planning, intervention, and evaluation of patient care
- Must have positive attitude with demonstrated ability to handle delicate situations and to communicate effectively with members of the hospice team, patients/families, and community agencies/institutions
- Must possess ability to work non-judgmentally with persons of diverse cultural, religious, and socioeconomic backgrounds and lifestyles
- Must be self motivated with strong organizational skills and demonstrated ability to prioritize tasks, handle pressure, and utilize critical thinking/problem-solving techniques
- Demonstrated ability to respond well to supervision and work independently with minimal direction and appropriate collaboration when required
- Ability to operate clinical equipment as well as office equipment such as computers, telecommunications devices, etc.
- Must have current Georgia driver’s license with satisfactory driving record and the availability of a reliable, licensed and insured vehicle for use on the job
- Must have current CPR certification
