Summary
Coordinates the annual contractual review process for Host Home Providers. Conducts the annual evaluation of physical environment and ensures that all other required credential and documentation are up to date and complete. Regularly audits Host Home Provider records. Assists in the initial orientation and ongoing education of Host Home Providers.

Essential Job Functions
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:
- Coordinates all steps in the annual review of contractual compliance and annual re-evaluation process.
- Conducts annual physical environment evaluation.
- Conducts periodic home visits and health and safety reviews.
- Regularly report to operations management on issues of contractual compliance.
- Provides information to the matching team on homes with some availability for placement.
- Maintains and audits Mentor files regularly for compliance with all regulatory, contractual, and internal company requirements.
- Works with management team to develop and implement a Mentor retention plan.
- Coordinates ongoing Mentor appreciation activities.
- Works with management team to develop continuing education sessions.
- Travels in-state 3-4 days a week to conduct on-site visits to Mentor homes.
- Performs other related duties and activities as required.

Minimum Knowledge And Skills Required By The Job
The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job:
Education And Experience
- Bachelor’s Degree in human services
- Two years of related human services experience preferred
- An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements
- Travel as needed
