Position Summary
The Case Coordinator provides organizational and programmatic support, including admissions assistance, direct participant support, care coordination, and communication with families, staff, and external partners.
Key Responsibilities
- Support and coordinate services to ensure participant needs are met.
- Assist participants with understanding their goals and programs.
- Serve as a liaison for families, providing updates and resources.
- Organize and lead participant conferences.
- Review evaluations, progress notes, and discharge summaries.
- Maintain accurate participant documentation.
- Assist with discharge planning and follow-up.
- Communicate case-specific updates with team members.
- Attend required meetings, including clinical meetings.
- Review monthly bills and report discrepancies.
- Coordinate participant incentive accounts with the Case Manager.
- Perform other duties as assigned.
- 🎓 Bachelor’s degree in a Social Services–related field required
- 🧑⚕️ Two years of experience supporting individuals with disabilities in a medical or rehabilitation setting
- 💬 Bilingual in Spanish preferred
- 🚗 Ability to travel as needed
- 💪 Light physical work (up to 20 lbs occasionally)
