- Provide individualized, family-centered support in homes, communities, and other outreach settings.
- Help caregivers build skills that promote child safety, stability, and well-being.
- Use Motivational Interviewing and trauma-informed approaches to engage youth and families.
- Support families in developing natural support networks and connecting with community resources.
- Assist with reunification, family visitation, and transitions into or within the home.
- Help youth and young adults develop skills related to housing, employment, finances, healthcare, self-care, and independent living.
- Connect youth with positive recreational, educational, social, and community activities.
- Collaborate with DCF caseworkers, providers, and other members of the family's support team.
- Maintain timely and accurate documentation in electronic health and state systems.
- Participate in supervision, team meetings, training, and ongoing professional development.
- Communicate regularly with DCF and other providers regarding service delivery, engagement, progress, and safety concerns.
- Serve as a mandated reporter and follow all applicable agency, contractual, and regulatory requirements.
