The Case Manager (Client Acquisition Specialist) is responsible for driving company growth by bringing in new clients/patients. Responsibilities
- Proactively identify and engage with hospitals, clinics, physicians, and community organizations to generate new patient referrals.
- Actively recruit and onboard new clients/patients for home health care services
- Build and maintain relationships with hospitals, rehabilitation centers, social workers, discharge planners, and community organizations
- Conduct outreach and marketing efforts in the community to generate referrals
- Educate potential clients and families on available services and benefits
- Track leads, referrals, and conversion rates to meet weekly and monthly growth targets
- Collaborate with internal team to ensure high-quality service delivery and client satisfaction
- Maintain strict compliance with HIPAA regulations by safeguarding patient information during all stages of case management and documentation.
- Track referral progress through detailed documentation of intake activities, patient service status, and discharge planning updates.
Qualifications
- Excellent communication skills for engaging with patients, families, healthcare providers, and referral sources effectively.
- Proven ability to meet or exceed growth or sales targets
- HS Diploma/College Degree is NOT required but it is preferred.
- Knowledge of Medicaid/Medicare and home care services is a plus
- Valid driver’s license and reliable transportation preferred
Pay: $800.00 - $3,000.00 per week
