SAGE is the country’s largest and oldest organization dedicated to improving the lives of lesbian, gay, bisexual and transgender (LGBTQ ) older people. Founded in 1978 and headquartered in New York City, SAGE is a national organization that offers supportive services and consumer resources to LGBTQ older people and their caregivers, advocates for public policy changes that address the needs of LGBTQ older people, provides education and technical assistance for aging providers and LGBTQ organizations through its National Resource Center on LGBTQ Aging, and cultural competence training through SAGECare. SAGE supports a growing network of 70 SAGECollab Partners across the country.This position will report to the Assistant Director of Care Management & Resident Services.The responsibilities: Provide case management, information, referrals, and advocacy to older adults; perform intakes/assessments, home visits, and provide case management to SAGE residentsConduct benefit and entitlement eligibility assessments, and assist clients with public benefits applications and other entitlement programs and opportunitiesConduct group workshops/programs/support groups as neededCollect and enter all participant and service unit data in database(s) thoroughly and timely, in accord with organizational policies and regulatory mandatesAssessing older adult needs and developing care plan strategiesProviding support and resources, good relationships with external providers and developing linkages to community resourcesResearch and provide referrals based on care plan goalsAssisting with benefits such as Access A Ride or Visiting Nurse services, Meals on Wheels, etc.Responding to crisis situations and developing follow-up report with details and recommendationsCommunicate with family members and other stakeholders involved in the residents’ care plan servicesPerform other duties as needed or assigned.While no one person embodies all the qualities outlined below, the ideal candidate will possess many of the following professional and personal abilities, attributes, and experiences. Familiarity with social and aging services networks and supports.Monday-Friday with the occasional weekend and/or overtime35 hours a week.
Salary : $22 - $29
