About the Hospital Liaison Role
As a Hospital Liaison within our County Programs team, you'll play a vital role in supporting individuals at Wernersville State Hospital (WeSH). This position is ideal for someone who is resourceful, compassionate and thrives in fast-paced environment.
Starting Rate: $23.651/hour, per current union agreement

Hospital Liaison Core Responsibilities
  • Request and/or complete authorizations for services
  • Assist in identifying and accessing community resources and providers
  • Verify MA status/insurance eligibility
  • Completed and update service plan
  • Complete referral to appropriate agencies and/or units within SAM, Inc.
  • Provide crisis intervention
  • Provide support to consumers
  • Participate in interagency planning
  • Attend court hearings
  • Participate in discharge/transition planning
  • Notify designated SAM staff of hospital admission
  • Be physically present on the acute care/WeSH hospital units
What You Bring
  • Bachelor’s Degree from accredited College or University in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation or related field as agreed to by management and the union
  • A combination of relevant experience and education considered.
  • Strong analytical, decision-making, and organizational skills
  • Commitment to person-centered, ethical practice.
  • A valid driver’s license and reliable transportation
  • Required clearances (Act 33, Act 34, FBI fingerprinting) and drug screening
  • Ability to lift up to 15 pounds and work in an office environment

Location & Schedule
This position is based at Berks County, PA. Work is primarily located at Wernersville State Hospital with some remote flexibility.
Office hours are Monday through Friday, 8:00 AM to 5:00 PM.
