About the Hospital Liaison Role
As a Hospital Liaison within our County Programs team, you'll play a vital role in supporting individuals at Wernersville State Hospital (WeSH). This position is ideal for someone who is resourceful, compassionate and thrives in fast-paced environment.
Starting Rate: $23.651/hour, per current union agreement
Hospital Liaison Core Responsibilities
- Request and/or complete authorizations for services
- Assist in identifying and accessing community resources and providers
- Verify MA status/insurance eligibility
- Completed and update service plan
- Complete referral to appropriate agencies and/or units within SAM, Inc.
- Provide crisis intervention
- Provide support to consumers
- Participate in interagency planning
- Attend court hearings
- Participate in discharge/transition planning
- Notify designated SAM staff of hospital admission
- Be physically present on the acute care/WeSH hospital units
What You Bring
- Bachelor’s Degree from accredited College or University in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation or related field as agreed to by management and the union
- A combination of relevant experience and education considered.
- Strong analytical, decision-making, and organizational skills
- Commitment to person-centered, ethical practice.
- A valid driver’s license and reliable transportation
- Required clearances (Act 33, Act 34, FBI fingerprinting) and drug screening
- Ability to lift up to 15 pounds and work in an office environment

This position is based at Berks County, PA. Work is primarily located at Wernersville State Hospital with some remote flexibility.
