CNA / Phlebotomist

Primary Purpose and Function

Responsible for attending to patient medical needs and assisting in the maintenance of a safe therapeutic 

environment.   

Required Qualifications

• Mustpossesshigh school diploma or GED equivalent; Associate’s or bachelor’s level degreepreferred 
• Mustpossessand maintain active Certified Nursing Assistant Certification or Nursing Assistant Registered in Washington State from accreditedprogram 
• One (1) year prior experience as a CNA/NARrequired  
• Mustpossessa valid WA state driver’slicense  
• Demonstratedproficiencyin the use of Microsoft Office and the ability to use various softwareprograms
• Must be able to successfully pass a comprehensive criminal background check (Federal and state)
• AFH Staff:  ability to successfully pass the Department of Social and Health Services backgroundcheck
• For positions involving driving personal vehicles or company vehicles as part of the job duties, validdriverslicense, proof of personal liabilityinsurance
• AFH or other assigned positions – successfully pass THIRA’s Driver Assessment
• Ability to work evenings, nights, weekends & holidays asneeded
• Perform other duties asassigned

Preferred Qualifications:

• Valid WA state driver’s license

• Prior mental or behavioral healthcare experience

Addendum A: Essential Functions and Responsibilities

• Assist THIRA nurse to create,maintain, and track patient health history and medical documentation in electronic medical recordssystem 
• Take vitals on patients daily and/or as needed; document accordingly in patient’schart 
• Perform initial new patient medical assessment, including but not limited to, reconcile medications, obtain medical/surgical history and most recent labs, confirm all patient allergies, and document all findings in patientchart 
• Maintain thorough medical/medication data for patients in EMR patientcharts  
• Observe and report changes in patient’s behaviors, physical symptoms, and mental health status to the Medical Team 
• Perform skin check on patients with a history of Non-Suicidal Self Injury (NSSI) and inform the team of any observed evidence of recentNSSI 
• Assistpatients with self-administered medication while on-site at program; documentaccordingly 
• Administer lab and genetic testing orders and enter prescription refill requests asneeded 
• Provide meal and snack supervision; document asneeded 
• Support andassistpsychotherapists, dietitians, art therapist, and milieu support therapist in creating andmaintaininga safe and therapeutic milieuenvironment 
• Maintain infection control standards within our facility, ensure employees and patients uphold procedures and standards asrelevant 
• Obtain urine specimens and send to lab for random patient drug screening, follow up asnecessary 
• Communicate with outside providers and patient family membersregardingmedical information asneeded 
• Support maintenance of patient illness tracking log and consult with patientsregardingquestions/concerns about presenting physical medicalsymptoms 
• Provide monthly audits of over-the-counter medications, emergency medications, and first aid kits to ensure that expired medications are disposed of andreplaced 
• Ensure maintenance of medical info binders for all required patients to be used for emergencysituations 
• Clean, sanitize andmaintainsafe nurse’sstation 

• Assistin coordination for the disposal of biohazard waste withappropriatevendors 
• Assistwith inventory and maintenance of medicalsupplies 

 RTF Essential Functions:

• Serve as point of contact for new patients entering the program orienting them to program structure,spaceandrules 
• Participate in activities with residents at the house; house meeting, cooking,games 
• Safely drive/transport patients to and from the Retreat at THIRA and facility  
• Accompany residents and therapeutic staff on recreational and therapeutic outings and ensure patientsafety 
• Complete rounds on patients at minimum of 1-hour time intervals, morefrequentlyif needed as clinicallyindicated. Document in daily review. 
• Support Compliance Department by conducting chart audits and relaying findings to Senior Compliance Coordinator 
• Facilitate start to program day and ensuring patients engage in designated morning hygiene timeappropriately 
• Facilitate morning medicationpass 
• Set up breakfast and hand off to Dayshift 

Addendum B: General Responsibilities & Expectations

• Support andmaintainpoliciesregardingprofessional ethics, includingappropriate boundariesand clientconfidentiality;monitordocuments according to company and HIPAA policies. 
• Report any immediate safety hazards to appropriate supervisory personnel or maintenance personnel.
• Adhere to WA State Requirements for Mandated Reporting  
• Demonstrate awareness and respect for diverse clients andstaff

• Adhereto THIRA Health’s compliance standards, Ethics and Code of Conduct Policy and Conflict of Interest Policy 
• Attend and activelyparticipatein all required THIRA meetings, trainings, and all other coverage, duties, and special projects asassigned 
• Ability to exercise discretion and judgement with sensitive and confidential matters, including PII, PHI, and HIPAArequirements  
• Possessstrong verbal and written English communicationskills  
• Demonstrated ability to multi-task
• Demonstrated ability to work in a fast-paced environment with limitedsupervision
• Maintain focus on THIRA’s core principles, priorities, and patient-centeredculture  
• Effectively communicate with all levels of staff, visitors, healthcare professionals, marketers, and the public in a professional, sensitive, and interpersonal manner 

Working Environment

Work is performed primarily indoors, with both artificial and natural lighting. Equipment includes standard office equipment, i.e. computer, telephone, fax machine, copier, printer, and scanner. Physical demands include sitting, walking, standing, bending over, repetitive use of hands, and lifting up to 25lbs. Position requires close contact with all THIRA Health staff and mental health patients. Hours and days can vary within a Sunday through Saturday week.
