Physical Therapist (PT) – Skilled Nursing Facility | Rochester, NY
$45–$53/hour
We are seeking a Physical Therapist to join our team at a Skilled Nursing Facility in Rochester, NY. This is an excellent opportunity to work in a collaborative environment focused on quality patient care.
Responsibilities
- Evaluate residents and develop individualized treatment plans
- Provide skilled therapy services to improve mobility, strength, and function
- Work closely with nursing, OT, speech, and facility staff
- Complete documentation in a timely and compliant manner
- Participate in care plan meetings as needed
Qualifications
- Active NY Physical Therapist license (or eligibility)
- SNF or rehab experience preferred
- Strong clinical and communication skills
- Ability to manage a caseload
