What are the responsibilities and job description for the Social Worker - Case Management position at Serenity Springs Recovery Center?
Serenity Springs Recovery Center is a 12 stepped based men's residential drug and alcohol treatment center. We are seeking an individual that can serve as a case management five days a week. Hours are 8 AM- 4 PM
Case Management- The Case Manager is responsible for coordinating client care, overseeing discharge planning, and ensuring proper aftercare arrangements. This role works closely with internal teams, external providers, and family/support systems to promote effective continuity of care.
Responsibilities Conduct assessments and develop individualized service plans Coordinate care with clinical, medical, and administrative teams Facilitate discharge planning and aftercare referrals Monitor client progress and update service plans as needed Maintain accurate and compliant documentation Communicate with community resources and treatment partners Support client transition into continued recovery post-discharge
