Duties
- Deliver comprehensive nursing care to residents with memory-related conditions, ensuring their safety and comfort at all times
- Administer medications, including injections and IV infusions, following prescribed protocols and maintaining precise medical documentation
- Conduct detailed patient assessments, vital signs monitoring, and physical examinations to inform personalized care plans
- Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and ongoing health coaching
- Utilize electronic health record (EHR) systems such as Epic or Cerner to document patient care activities accurately and efficiently
- Manage acute pain situations and respond promptly to medical emergencies within the memory care setting
- Assist with activities of daily living (ADLs), including tube feeding, catheter care, and mobility support using equipment like Hoyer lifts
Qualifications
- Valid LPN license in good standing with current state licensure requirements
- Prior experience working with individuals with developmental disabilities or in Alzheimer’s/dementia care environments
- Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques
- Experience with EMR/EHR systems such as Epic or Athenahealth for accurate medical documentation
- Demonstrated ability to perform patient assessment, vital signs measurement, medication administration, and basic life support (BLS) certification
- Critical care experience preferred, including hospital or ICU settings such as Level I Trauma Centers or Emergency Rooms
- Familiarity with infection control practices, hospice care principles, and acute pain management strategies
