Job Description
The Mental Health Practitioner will provide culturally competent mental health services (skills training) to adults and/or kids diagnosed with mental health conditions as documented by a comprehensive diagnostic assessment. Skills training is provided primarily in the client's home or in the community. This position is an independent position where you primarily work out of your client's homes or in the community. The schedule is very flexible- you are in charge of your own schedule. This company is based out of Hutchinson, but we offer services in throughout McLeod County.
Summary of essential job functions
- Secure client information per HIPAA standards
- Maintain case files that are up to date
- Review diagnostic assessment completed by a MH Professional prior to initiating services
- Develop and implement a culturally appropriate treatment plan and crisis plan for each client
- Track and ensure that all necessary paperwork is current and compliant
- Review treatment plans with consulting MH Professional
- Administer Functional Assessments as required
- Maintain progress notes
- Work closely with counties, psychiatrists, therapists, medical professionals and other community service providers in coordinating services by following coordination of care procedures
- Attend, at minimum, one clinical consultation group per month (unpaid).
- Participate in agency quality improvement activities
- Complete 10 hours of continuing education every calendar year
- Other duties as assigned
Minimum requirements
Must meet Mental Health Practitioner criteria, per MN statute (minimum of 2000 hours of supervised work with adults or kids in a mental health setting). Must be proficient with computer, email, and Microsoft applications. Must complete 30 hours of training prior to starting this position.
Abilities required
Must have ability to travel (reliable car with insurance and valid drivers license) and sustain attention to detail.
