The CHW will play a crucial role in improving the health outcomes of community members through education, advocacy, and support.
- Outreach and Engagement: Conduct outreach activities to engage community members, including home visits, workshops, and community events.
- Health Education: Provide culturally sensitive health education on HIV/STI prevention and general health promotion.
- Care Coordination: Assist clients in navigating the healthcare system, including scheduling appointments and connecting them with necessary services.
- Advocacy: Advocate for clients to ensure they have access to public health programs and resources.
- Support Groups: Facilitate support groups and educational sessions on relevant health topics.
