We are seeking a compassionate and detail oriented social worker to join our experienced hospice team serving patients and families throughout the greater Boston and surrounding areas.
Position Purpose
Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group.
Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high-risk for suicide, neglect or abuse and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provides crisis intervention when necessary.
Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of psychosocial stresses on the symptoms of the terminal illness.
Education required: Bachelor’s degree in Social Work or related field
Professional licensure required: Massachusetts LCSW or LICSW. Responsible for annual re-licensure per federal and state regulations.
Social Worker must communicate effectively with residents, families, visitors, and interdisciplinary team to support the emotional, mental, and clinical well-being of residents.
Experience in long term care social work preferred, as well as knowledge of Medicare and Medicaid, but not required.
