Serenity Hospice is seeking a dedicated and compassionate Field Support Specialist to support and supervise our Hospice Home Health Aides (HHAs) in facility settings. This non-clinical position plays a key role in ensuring quality care, adherence to policies, and effective communication between HHAs, families, and the care team. The ideal candidate will have experience in hospice or home health care, strong interpersonal skills, and a commitment to excellence.

This role offers a flexible full-time schedule (7A-3P) with rotating weekends — one weekend on, one weekend off — making it a great opportunity for those who value work-life balance.

This position requires travel throughout Monmouth & Ocean Counties.

Responsibilites:

  • Oversee and evaluate the performance of Home Health Aides at assigned facilities.

  • Ensure compliance with hospice standards, policies, and procedures.

  • Provide training and ongoing guidance to HHAs on hospice protocols and best practices.

  • Identify performance concerns and improvement areas, and report findings to management.

  • Act as a liaison between HHAs, nursing staff, and management.

  • Maintain clear and compassionate communication with families through coordination with RNs.

  • Verify that HHAs adhere to scheduled start and end times.

  • Ensure compliance with break and meal period policies.

  • Monitor proper uniform and ID badge usage among HHAs.

  • Maintain detailed documentation of field visits, evaluations, and training sessions.

  • Prepare quality assurance and compliance reports for leadership review.

  • Offer real-time support and problem-solving assistance to HHAs in the field.

Requirements:

  • Previous experience in hospice care, home health, or a related healthcare setting.

  • Strong communication, leadership, and interpersonal skills.

  • Ability to establish rapport with HHAs and facility staff.

  • Self-motivated, well-organized, and capable of managing multiple responsibilities.

  • Valid driver’s license and reliable transportation required — frequent local travel is necessary.
